Nightdive Studios resurrects the 2002 third-person horror shooter by introducing a sequel to the events in Outpost 31. Players play as Captain J. F. Blake, the commander of a U.S. Special Forces rescue team sent to investigate the aftermath of a scientific expedition.

The gameplay focuses on confronting shapeshifting alien creatures in a challenging environment. Team management becomes one of the main mechanisms in which players must navigate complex interpersonal dynamics and the potential infection of team members.
